This is what I'm able to determine...
 
It appears Mark Hay attempted to purchase for himself a 2007-01:Retailer 2007 Membership Badge and a 5000:Official GTS 2007 Texas Hold 'Em Tournament ticket.
 
It also appears he attempted to purchase for Philip Rangel a 2007-02:Additional Retailer Badge.
 
A Transaction (XID # 20117684) was completed on 2/11/07 at 14:54:38 for $250.00 for the purchase of these three items.
 
When reviewing the items for Mark Hay and Philip Rangel I see the 2007-02:Additional Retailer Badge for Philip Rangel in our system. I see two incomplete transactions and no completed orders for the 2007-01:Retailer 2007 Membership Badge and the 5000:Official GTS 2007 Texas Hold 'Em Tournament yet it appears he was charged for all three items.
 
So... any idea what's happening and why? This is just one case.

If you're seeing no completed orders, then how do you know he was charged? Also, do the three things add up to $250 if purchased, or is $250 just the cost of one of them. Keep in mind that I know nothing about your data or your events. What do the numbers mean in front of each of the badge names? Is that in any way relevant to the problem, or should I ignore them?

Also, in addition to the answers to the above questions, please fill out a login request so I can log in and see what this looks like in your site. It's very hard to visualize without context.

I can see in iTransact that he was charged as we were going through to clean up uncomplete transactions.

Yes, the three items add up to $250.

The numbers are the SKU.

This is definitely a bug. Please open a bug report. Also please post back here with a link to the bug report if you'd like us to use some of your 10 support hours to make this a priority bug fix.

I was able to reproduce on latest SVN, so this problem still exists.

It seems to happen when a user adds a badge to their shopping cart and logs out before completing the transaction.

The next time they log in and try to make a purchase, they're charged for their old merchandise but the EMS isn't aware that they finally purchased it.

I'm going to start tracing to find a fix for this.

Yes - there should be a history on that issue because we've discussed it before. I swear it was supposed to be fixed though...

I think what you are referring to is an ongoing problem was was supposedly fixed (but now we see it isn't) but not necessairly what we are reporting now. Maybe they are linked though. Not sure.
