This is a tabForm template that uses fieldset, legend and labels to improve accessibility. It turns the tabs into rows of fieldsets. 

There are some inline styles included to give the form a nice clean and organized look.  The thumbnail attached uses our typography CSS which is not included but the form still looks good without it.

Mac: Firefox 2 IE, 5.2, Opera7.5.4, Safari 2
Windows: Firefox 2, IE 6, Opera 9.2
